Healthcare SEO Case Study: How Alien Road Took Liv Hospital from 30K Visits to 4.4 Million Monthly Clicks

Liv Hospital is an internationally accredited hospital group founded by MLP Care. Its flagship Ulus branch opened in Istanbul in 2012; today the group runs eight hospitals across Istanbul, Ankara, Gaziantep, Samsun and Baku. Clinically it was already world-class. Online it was not: about 30,000 monthly visits, hundreds of high-intent medical searches where it did not appear in the top 100, and international patients quietly going to smaller competitors who had learned the digital game first.

Over 14 months we rebuilt the hospital’s digital infrastructure from the ground up and tied content, SEO, design, software, advertising and reputation into one revenue engine. The outcome: monthly organic clicks grew from ~30K to a peak of ~4.4M, the site ranked for ~2 million U.S. keywords with 270M+ monthly impressions, the average treatment package rose from $300 to $13,000, and the hospital’s Google rating climbed from 3.8★ (900 reviews) to 4.8★ (9,400 reviews). This is that story, told step by step.

The 60-second version

The challenge: a world-class hospital, nearly invisible in search

The audit found six problems, and none of them could be fixed with advertising alone.

  • The site described the hospital, not the patient’s question. There was no systematic answer for each step of the clinical journey — symptoms, condition, diagnosis, treatment, recovery, medication, follow-up.
  • No scalable structure. Content lived as scattered standalone pages: no topical authority, weak internal linking, every page fighting on its own.
  • Single-language content for global demand. Nothing answered the country, city, treatment and price combinations international patients actually search.
  • A heavy, slow foundation. A page-builder plugin dragged down Core Web Vitals and muddied the structured signals search engines read.
  • The wrong patients. Most enquiries were worth around $300 — consultations and entry-level services — while the hospital’s real strength was complex treatment journeys worth far more.
  • No system, no tracking. Leads arrived by web form, phone and above all WhatsApp, then disappeared into inboxes and phones. Nothing connected a Google click to an actual surgery; marketing was flying blind.

We also chose the competition deliberately: not local hospitals, but the top U.S. clinics that had held the first page for years. The goal was not to be visible; it was to compete with the best.

Five goals, written in month one and still binding in month fourteen

  1. Be there for every search. Whatever a patient searches about their treatment, Liv should meet them — 350K+ pages covering the entire medical journey.
  2. Compete with the best. Build authority that rivals the leading U.S. clinics, not the hospital down the road — ~2M U.S. organic keywords.
  3. Play by the rules. Strict E-E-A-T discipline: no shortcuts, no black-hat tactics, no thin automated content.
  4. Win the AI era. Visibility in AI-generated answers (AEO, GEO, AIO) as a day-one priority, not an afterthought.
  5. Build a lasting asset. Infrastructure the hospital owns and that keeps working after the campaign ends.

The strategy: three layers, one revenue engine

System first, campaign second. Each layer sets up the measurement the next one needs; you cannot fix a leaking pipeline with more traffic.

  • Layer 1 — Infrastructure: custom CRM, WhatsApp Cloud API and offline conversion, so every click can be tied to real revenue.
  • Layer 2 — Demand: the SEO authority engine, content production and paid launch, bringing high-intent traffic in the right market and language.
  • Layer 3 — Compound: Web Stories, digital PR, CRO and reputation, so results keep growing month over month.

Timeline: 14 months in four phases

  • Phase 1 · Discovery (month 1). Technical SEO audit, five-angle competitor analysis, UX and CRO audits, reputation audit — before a single dollar of media. Finding: for hundreds of high-intent medical keywords, Liv was not even in the top 100. The WhatsApp leak was identified and the roadmap approved.
  • Phase 2 · Foundation (months 2–4). Custom CRM built from scratch around the patient journey (enquiry → consultation → quote → treatment → follow-up); WhatsApp Cloud API feeding every conversation into it; offline conversion linking a click to a surgery. 3,200+ WhatsApp conversations a month came into view; first-response time fell from ~48 hours to under 15 minutes.
  • Phase 3 · Ramp (months 5–9). A clean-slate decision: the entire English/global engine on a purpose-built subdomain, int.livhospital.com, launched 15 September 2025. Programmatic URL structure for 43 countries, 4,755 clinical pages, 86K blog articles, four specialist verticals; Google Ads live; social identity shipped. Average package $300 → $8,400; organic 30K → 850K a month.
  • Phase 4 · Compound (months 10–14). 554 Web Stories, link building and digital PR (940 referring domains, Domain Rating 38 → 67), weekly CRO experiments, reputation at scale. Average package reached $13,000; ROAS stable at 7.4×.

The turning point

In month two, paid traffic was rising but revenue was not following. Offline conversion data showed why: the best patients were not converting through the form. They were converting on WhatsApp, days later, after a long decision process — and we had been optimising for the wrong event. We rebuilt the campaigns and feeds around a WhatsApp-first, long-consideration journey. That is the moment the average package started climbing from a few hundred dollars to five figures.

Software: a core competitors cannot copy

Four systems, built over 14 months and run as one backbone: a patient-journey CRM with lead ownership, SLA rules and automation; WhatsApp Cloud API ingestion with tagging and automated routing; an offline conversion mechanism that walks a treatment back to the click; and an internal project-management panel visible to every team. Stack: React and Next.js on the front, Node.js with REST and queues behind, PostgreSQL, Redis and S3 for data, WhatsApp Cloud API and Google Ads API in the loop.

  • Six releases from the private CRM launch (Nov 2025) to offline conversion v3 with multi-touch attribution (Jun 2026), including predictive lead scoring that flags high-value treatment intent within the first 60 minutes.

Web design: the experience that turns a ranking into a patient

Rankings bring patients to the door; the experience decides whether they come in. Two designers, twelve weeks, medical UX: patient interviews mapping the cross-border decision, treatment funnels and wireframes, clinical design tokens, hi-fi prototypes with treatment configurators, a Next.js front end with WhatsApp handoff, and a weekly CRO cadence. The custom WordPress build replaced the theme; clinic-specific components, a distraction-free blog layout, redesigned check-up pages, interactive treatment calculators, automatic table of contents, a medical-editor author bar, an AI summary bar and always-visible appointment buttons followed. WCAG 2.2 AA compliance, Core Web Vitals passing with LCP at 1.6 s.

Brand: trust you can see

Liv had no guideline for its own brand, and it showed. We rebuilt the logo from scratch, defined the identity — primary #1C92E9, deep #236791, secondary #DBECFD, positive #16AC4E — and wrote a four-pillar voice: sincere-clinical, proven by numbers, a guide not a salesperson, and international plain English. The identity was applied to every surface: website, Pinterest, blog infographics, seven social platforms, ad creatives, email templates, WhatsApp broadcast images, print and video. Branded search volume rose 212% and direct traffic 96%.

SEO: the authority engine at scale

Our thesis was simple: you do not win medical search with a few hundred good pages. You build an engine that covers the entire medical journey, at scale, with clinical accuracy and editorial discipline. More than 50 specialist editors, every article approved by a medical editor before publication.

  • 4,755 interlinked clinical pages. A clinical taxonomy of 26 specialties and 711 subcategories, five child pages under each, and two templates: procedure-focused (overview → indications → diagnosis → treatment → recovery) and condition-focused (overview → symptoms → diagnosis → treatment and rehabilitation → lifestyle and prevention).
  • 86,000+ blog articles. The question-driven upper funnel — every condition, treatment, symptom, medication and recovery path — produced through a repeatable line: taxonomy and AI keyword research → verified brief → FAQ-structured article → medical editor approval.
  • 255,000 programmatic country pages. A four-part URL architecture — country / city / topic / subtopic — across ~297 locations in 43 countries, each page unique, with a pricing layer of ~1.28 million price points and cross-market comparisons.
  • Four expert verticals. Medical technologies, medications, check-ups and long-form health guides, all interlinked with the clinical pages and blog.
  • 554 Web Stories across 22 clinical categories: from ~11 monthly clicks in October 2025 to a peak of ~422 in January 2026, ~15,400 views, one story alone ~438 clicks.
  • A platform built from scratch. The heavy page builder was removed, a lightweight widget system and a no-code visual editor built for the content team (3–4× faster production), hundreds of pages migrated automatically with zero content loss and a generated redirect map. LCP 1.6 s, INP passing, CLS under 0.05, schema.org throughout, canonicalisation and XML sitemaps at scale.

Reach, Google Search Console, peak month June 2026: 140×+ organic clicks (~30K → ~4.4M a month), 270M+ monthly impressions across Search, Discover and News, ~350K pages in the engine, ~2M U.S. organic keywords per Semrush with strong parallel footprints in the U.K., Canada, Australia and India. None of it was bought; it is an owned asset that keeps compounding after the campaign.

Paid media: high intent, high value

Waste was cut on day one and the account rebuilt on a SKAG structure with a deep negative pool. Search in English and Arabic plus other markets, Shopping with package-and-price feeds per market through Merchant Center, and YouTube in-stream and Discovery for awareness and retargeting — all optimised on offline conversions rather than form fills. ROAS climbed 1.8× (month 1) → 2.9× → 4.3× → 5.8× → 7.4× (month 14); cost per conversion fell 58%; qualified leads rose 214%; ad-spend efficiency improved 311%.

Social, content, CRO, AI visibility and PR

  • Social media: seven platforms under one visual language and voice, ~150 posts a month, community management with a ~2-hour response time — 268K followers, 4.6% engagement.
  • Content marketing: treatment-intent clusters with pillar pages and supporting guides, 340+ articles across 24 pillar categories, 3:40 average reading time, +287% organic traffic on the editorial programme, ~180 email leads a month from nurture flows.
  • CRO: end-to-end audit, weekly A/B and multivariate tests, Hotjar and GA4 behaviour analytics, landing-page work worth a 3.2× conversion lift, and a six-step funnel from landing page to treatment.
  • AI visibility: citation frameworks for ChatGPT, Gemini, Perplexity, Claude, Copilot and Meta AI; FAQ blocks on every article; llms.txt live; medical bylines, update dates and disclaimers on every page. When Google moved to AI-powered search, Liv rose instead of falling.
  • Digital PR and authority: 48 media placements, ~180 broken-link reclaims, monthly unlinked-mention sweeps, quarterly backlink audits, and a reputation programme that took the rating from 3.8★ to 4.8★.
  • Measurement: GA4, Google Ads, CRM, WhatsApp, Merchant Center and call data in one layer, offline and multi-touch attribution, a role-based live Looker Studio dashboard, monthly reports and quarterly business reviews.
